Capstone Dental Discount Plan:
Our in-house discount plan is an annual reduced fee option that allows individuals and families to receive quality dental services from without many of the restrictions of an outside dental insurance plan.
The Capstone Dental Discount Plan includes the following at no additional charge:
Adults: Two Periodic Examinations per calendar year Two Dental Cleanings (regular prophylaxis or periodontal maintenance) per calendar year One set of BWXs and two PAXs per calendar year One Panoramic x-ray every three years One Fluoride Treatment per calendar year
Children to age 18: Two Periodic Examinations per calendar year Two Dental Cleanings per calendar year One set of BWXs and two PAXs per calendar year One Panoramic x-ray every three years One Fluoride Treatment per calendar year Sealants (one per tooth per lifetime)
Basic and Major services receive a 20% discount off the usual and customary fees.
Additional Cleanings: Prophylaxis (cleaning) - no x-rays/exam $80 Periodontal Maintenance - no x-rays/exam $90
Terms and conditions:
Annual enrollment: $300 a year—Collected in full at the time of your visit which the plan will be active for one year and will not actively renew. You will need to pay the fee following the plans 1 year expiration date on the day of your next appointment if you would like to continue the plan.
Cancellation Policy: Once you pay the annual enrollment fee we are not able to offer you a refund.
