How to Request an Appointment

To request an appointment , please click on the following link below in light blue "Book your appointment now!", to schedule an appointment.  

DO NOT USE THE PORTAL TO SCHEDULE ANY TREATMENT WE HAVE PRESENTED TO YOU IN THE OFFICE.

After clicking on the link- If you are a New Patient, make sure under Patient Type  that "New"  is light blue and select under Reason either-  "New Patient/DR ONLY- POSSIBLE SAME DAY TREATMENT " if you are in pain or have an issue with a tooth that needs immediate attention, or you can select "New Patient /Clean Exam/Xrays " if you would like a routine check up and establish being a New Patient at Capstone Dental. 

If you are an Existing patient under  Patient Type  make sure that "Existing" is light blue and select under Reason either- "Broken or chipped tooth, Discomfort/Pain, Problem with Crown or Bridge. or Routine exam/cleaning. " - based on what you would like to be seen for you may schedule with a specific provider as well.  If you are an EXISTING PATIENT and have not been seen in over a year please call us for an appointment. 

(PLEASE NOTE-If you are in pain and there is no appointments available please give us a call or you can text us and we can maybe squeeze you sooner)

ONCE YOU SCHEDULE YOUR APPOINTMENT PLEASE BE AWARE THERE IS A $75 FEE MINIMUM IF YOU CANCEL WITHIN LESS THAN 48 HOURS.
