Appointments
We have made it easy to request an appointment through our website. If for any reason you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible. Out of respect for the staff and other patients, late arrivals of 15 minutes or more generally will not be allowed to keep their appointments. Cancellations made less than 48 hours before an appointment may be subject to a last-minute cancellation fee. See the Appointments section of our Office Policies (New Patient Form) for details.

(As of 1 January 2024, we no longer accept Anthem Blue Cross & Blue Shield, Guardian, or United Concordia insurance plans. We are not contracted with Medicare or Medicaid.)
Capstone Dental does not participate in any HMO (Health Management Organization) or EPO (Exclusive Provider Organization) plans. We will submit claims to in-network dental insurances for you at no charge. Patients who wish to submit any unpaid charges to their medical plan or an out-of-network dental plan for reimbursement must do so themselves, but we will be happy to provide any necessary documentation upon request.
All co-pays are due at the time services are performed. Patients who are unable to pay in full must coordinate a payment plan with the front office prior to receiving treatment. Payment plans require automatic credit or debit card payments.
